What Panchakarma actually means
Pancha means five and karma means action. The term refers to five named procedures — Vamana, Virechana, Basti, Nasya and Raktamokshana — each of which removes a specific category of accumulated waste from a specific route out of the body. Oil massage, steam and Shirodhara are not among them. They are preparatory or supportive therapies, valuable in their own right, but a course consisting only of those is not Panchakarma.
This distinction matters commercially as well as clinically. Across Hyderabad the word has been attached to three-day wellness packages priced for tourists. Those may leave you relaxed. They will not shift entrenched psoriasis, reverse a fatty liver, or take the inflammation out of a rheumatoid joint, because the mechanism that does that work — mobilising toxins into the gut with days of internal oleation, then expelling them in one controlled procedure — was never carried out.
A genuine course is also not something you should be sold on arrival. Roughly four in five patients who come to us for chronic conditions begin on medical management — herbal formulations, diet and lifestyle correction — and never need Panchakarma at all. It is indicated when the disease is deep-seated, recurring, or unresponsive to medication, not as a default.
The five procedures, and what each one treats
All five are performed in-house at our Boduppal hospital.
Virechana
Therapeutic purgation that clears Pitta and metabolic waste through the digestive tract. Our most-prescribed procedure — used for psoriasis and stubborn skin disease, fatty liver, chronic urticaria, migraine and IBS. Preceded by 3–7 days of internal oleation (Snehapana) with medicated ghee, which is the part most centres skip.
Basti
Called the "half-treatment" of Ayurveda because of how much it addresses alone. A course of medicated oil and decoction enemas, given in defined sequences (Kala, Yoga or Karma Basti), for lumbar spondylosis, sciatica, osteoarthritis, neurological conditions and infertility.
Nasya
Medicated oils and powders administered through the nasal passage to treat everything above the collarbone — chronic sinusitis, allergic rhinitis, cervical spondylosis, migraine, hair fall and early cataract. Usually a 7–14 day course alongside steam.
Vamana
Physician-induced therapeutic emesis for entrenched Kapha disorders — bronchial asthma, chronic rhinitis, obesity with hypothyroidism, PCOS and recurring skin conditions. Requires admission and close monitoring on the procedure day, which is why it is rarely offered properly.
Raktamokshana
Targeted bloodletting, most often by leech therapy (Jalaukavacharana), for varicose veins, non-healing diabetic ulcers, localised eczema and post-thrombotic complications where impure blood is the driver.
What a course actually involves
Four stages. The middle one is the shortest — and the one most places sell on its own.
Stage 1 — Assessment (Day 0)
A 60–90 minute consultation: full case history, Nadi Pariksha, review of your existing scans and blood work, and AI-assisted Prakriti mapping. Not everyone needs Panchakarma. Around 80% of our patients start on medical management instead, and we will tell you if that is you.
Stage 2 — Purvakarma (3–7 days)
Preparation. Internal oleation with graded doses of medicated ghee, external Abhyanga and Swedana to mobilise toxins from deep tissue into the digestive tract. Skipping or shortening this stage is the single most common reason Panchakarma fails to hold.
Stage 3 — Pradhanakarma (1–3 days)
The main procedure itself — whichever of the five your condition calls for — carried out under direct physician supervision with vitals monitored throughout.
Stage 4 — Paschatkarma (7–30 days)
Recovery and consolidation. A staged diet protocol (Samsarjana Krama) rebuilds digestive fire, followed by Rasayana herbs. This is where the result is locked in — and where a 3-day "detox package" leaves you worse off than before.
On duration: the reason a real course runs 7–21 days is that Purvakarma and Paschatkarma cannot be compressed. Internal oleation is dosed upward over several days until a clinical endpoint is reached; the post-procedure diet is rebuilt in stages over a week or more. Remove those and you have removed the treatment.

Frequently asked questions
How is this different from a Panchakarma package at a spa or resort?
A spa gives you oil massage and steam, which are two preparatory steps of Purvakarma — pleasant, but they are not Panchakarma. Actual Panchakarma is one of five specific purificatory procedures, prescribed after diagnosis, preceded by internal oleation, performed under physician supervision, and followed by a staged diet recovery. If nobody took your case history and no doctor is present during the procedure, you are having a massage.
How long does Panchakarma take in Hyderabad?
A clinically meaningful course runs 7 to 21 days depending on the procedure and your condition. Virechana typically needs 10–14 days end to end; a Basti course can run 8, 16 or 30 days. Anything advertised as a 3-day Panchakarma is skipping either the preparation or the recovery, and usually both.
Do I need to be admitted, or can I come daily?
Most patients come in daily as outpatients and go home afterwards. Vamana and the main day of Virechana are the exceptions — those require you to be at the hospital for the full day so vitals can be monitored. We plan the schedule around your work commitments where the protocol allows.
What does Panchakarma cost in Hyderabad?

Is Panchakarma safe if I am on allopathic medication?
Usually yes, and you should not stop your existing medication on your own. Bring your current prescriptions and recent reports to the consultation. Certain procedures are modified or deferred for uncontrolled diabetes, cardiac conditions, pregnancy and a few other situations — that assessment is exactly what the first consultation is for.
Will one course cure my condition permanently?
For an acute presentation, often a single course plus follow-up medication is enough. For chronic autoimmune and metabolic conditions, Panchakarma resets the ground and the maintenance is done through diet, lifestyle and Rasayana medication over the following months. Some patients repeat a shorter course seasonally. We will give you a realistic timeline at the outset rather than after you have paid.
